Since the willingness to provide prescriptions and the associated fees vary by doctor, please call ahead to confirm. Your prescription should include the following information: prescription strength, astigmatism power (not needed if no astigmatism), astigmatism axis (not needed if no astigmatism), pupillary distance, and multifocal add power (not needed if no presbyopia).
